MEDICAL RECORDS

Patients can access their medical records for FREE via the Patient Portal! Provide the front desk with your email address upon check-in to gain access to the Patient Portal.

If patients would like their records sent to them, there is a processing fee that will need to be paid prior to receiving your records:

Forwarding Records by Email: $6.50

Printing/Mailing Records: $12

Processing fees can be paid using a credit card, cash, or check.

To request medical records, please fill out the Disclosure for Protected Health Information form and fax or email it to us at medrecs@ptcoa.com.
