Payment Options

All co-pays and deductibles are due at the time of service. For your convenience we gladly accept cash, check and credit card/debit card. You must have your insurance card at time of check-in.


Cancellation Policy:

PTCOA understands that emergencies can arise. Click the link to view our Clinic Rules & Cancellation Policy. Please make sure if you are unable to keep your appointment that you allow 24 hours advance notice. Failure to provide advanced notice will be considered a “no-show” and the “no-show” fee will be applied. These fees must be paid at your next office visit in order to be seen. We apologize for any inconvenience in advance.

Insurance Coverage:

PTCOA accepts all major insurance carriers (including medicare, medicaid and all government expansion program insurances). We also will pre-certify any insurance requirements as a courtesy for our patients. Please have all insurance cards and copays ready at each and every visit. If we are unable to verify your insurance information then you will be expected to pay full cash price for services rendered.
